- Secondary Waiver – Required to be completed by all adults and athletes that will participate in any way in the tournament.
- Keller ISD Facility Waiver – Required to be completed by all adults and athletes that will enter into the Keller Natatorium
- Table Worker Form – Only the coach can complete this form submitting the names of the two experienced table workers who are allowed to attend games in order to work the table.
